Consistent Transcription Failing and Silent Audio (fixed by restart)

I keep running into errors where transcription takes a really long time and then fails. If I try to redo or restart the transcription in Monologue, it doesn't work; it just always times out. If I quit the app and open it back up, it's already transcribing in the background and is immediately transcribed.

The same thing happens with silent audio. I'll have silent audio, seemingly randomly, probably every five or so uses. Then when I quit the app and restart it, it immediately starts working again.
