Random Dictionary Words Injected Into Output

This is similar an issue that was marked as completed 6 months ago. I added it to that item and added this since the previous was marked as completed.

Monologue occasionally dumps seemingly random words from its learned dictionary/vocabulary into the transcription output. These are words I've never said — they appear to be pulled from the custom vocabulary or context features and inserted where they don't belong.

Steps to reproduce: Not consistently reproducible, but happens frequently enough to be noticeable — roughly every few sessions.

Expected: Transcription should only contain what I actually said, cleaned up for filler words and grammar as normal.

Actual: Random words appear in the output that I never dictated. They seem to come from the app's learned vocabulary or dictionary rather than from my speech.

Device: iOS and iPad OS 26.4 - both betas and final released version

Frequency: Intermittent but frequent — multiple times per day with heavy use

Severity: Medium-High — requires manual editing to fix, which slows down the workflow Usage: Top 7.2% of users after 2 weeks
